Expand Access to Healthcare

Project Description

This project aims to expand access to behavioral health & medical care for low income individuals and families. PHC seeks to open doors to preventative and chronic care, address health disparities, and improve health equity in our community. Social drivers impact the overall health and well-being of most of our patients. Examples include homelessness or unstable housing, food insecurity, access to education, language barriers, poverty and transportation.
